Invitation Request Form

To obtain an invitation to get your visa you will need to:

A) Book or reserve a ticket with a travel agent. We suggest Barbara Garcia as she is our travel agent and helps many travelers to Russia (BarbaraHouplinGarcia@AAAwin.com or 1-866-756-1049).  Reserving a seat does not guarantee the cost.

B) Send a scanned copy of the picture page of your passport by email to Fr. Myron (myron@catholic.vladivostok.ru)
 

C) Send the completed form below to Fr. Myron:

Approximate trip costs:

Visa - $70.00

Host fees, Transportation, Translators - $125.00

Room - $15.00 per day, Board - $15.00 per day for those who live in the parish premises.
